2. Sign up for free-Create free account
3. Click: Create Form
4. Click: Import form
5. Select: From a webpage
6. Paste link of To Your Wellness survey into the URL (https://www.jotform.com/build/251775119668166 )
7. Edit form how you want, change the name of the form by clicking Copy of Wellness Survey at the top and typing in whatever title you want
8. When you’re done, click Publish at the top
9. Quick share
10. Copy link
11. Save link to your notes or as a keyboard shortcut or on a personal BOARDS so you can text it out
